What is an anal fissure?
An anal fissure is a small rip or tear in the lining of the anal canal
Causes
- Chronic constipation and habitual straning
- Long standing condition of loose stool
- chronic laxative abuse
- Previous anal surgery/child birth/penetration
Site
Midline –
- Posterior – commonly seen
- Anterior – more common in female(3rd trimester preg and post paturm peroid)
Lateral –
- For other disease process
- Crohns disease
- HIV/AIDS
- Anal carcinoma
Sign and Symptoms
- Pain(Dyschezia)- starts with the passage of stool and last several minutes to a few hours
- Burning or itching with pooping
- Bleeding with bowel movement
- Experince spasm in the ring of muscle at the end of anus
- Change bowel habit-tendency
- Nervous manifestation-irritability, abdominal discomfort, digestive disturbances

Local examination
- Inspection- sentinal tag are usually visible
Generally parting the margin of anus the lower end of fissure can be seen.
- Digital examination- Acute-not done ,Chronic-button hole can be palpated
Anoscopy to demonstrated the other condition in chronic cases
